The knowledgeable staff at Dr. Kish and Associates are experienced in working with vision insurance providers. We will work directly with you and your vision insurer to submit claims regarding your vision care. We strive to ensure that you understand your insurance coverage and will answer any questions you may have regarding your benefits. Many current vision plans are designed to cover most of the basic vision-related costs, but not all costs associated with eye care. Most plans often require a co-payment for an office appointment as well as additional costs that are covered by the insured patient.

It is the responsibility of the patient and/or insurance card holder to present accurate and current insurance information. We request that you bring both your vision and medical insurance to each visit so that we may review or update our billing system. If you fail to bring current insurance information, the services will be considered self-pay. You will be asked to pay for the service on the day of the service. We accept cash, debit and credit cards and Care Credit as payment.
We are contracted with the following vision plans:
Sooner Care, Eyemed, Humana Vision, Superior Vision, Vision Benefits of America, United Health Care Vision/Spectera, Davis Vision, Avesis and NVA.
We are contracted with the following medical insurance carriers:
Medicare, Sooner Care, Aetna, BCBS, Cigna, Health Choice, Health Smart, United Healthcare, webTPA
It is your responsibility to understand the insurance companies with which we have contracts. If we do not have contracts with your insurance company you may be subject to out of network benefits which will result in higher co-payments, deductibles or non-covered care.
In addition, it is also your responsibility to understand your healthcare benefits with your insurance carrier. Please understand that there may be
co-payments, deductible and co-insurance requirements. In addition, your policy may not cover all services provided by our practice. As such, you may be asked to sign a financial waiver that indicates that you will take financial responsibility for services provided if your insurance company does not cover the service.
Co-payments are expected prior to being seen during your check in process. We accept cash, debit and credit cards, and Care Credit as forms of payment. If you do not bring your co-payment at the time of service you will be asked to reschedule your appointment to a time in which payment can be made.
We accept all major credit cards, debit cards, cash and Care Credit as forms of payment. We do not accept checks.
FLEXIBLE SPENDING ACCOUNTS
Many employers are offering Flexible Spending Account (FSA) options to employees. Sometimes these are referred to as cafeteria plans and are elective, supplemental insurance savings plans. These plans are designed to let you save money in an account, pre-tax, to pay for additional medical expenses such as eye exams, glasses, contacts, and often laser vision surgery. Check with the benefits administrator at your work to see if you are eligible for this program. FSAs can typically be used in conjunction with any vision insurance plan to offset any out-of-pocket costs on your behalf.
